We minimise the psychosocial and physical impacts of
heart conditions on Australian families.
Individuals affected by heart conditions have one of the highest per capita death, hospital admission and readmission rates in Australia.
This is often due to the lack of social support and information.
We believe Australians affected by heart conditions require a positive health enhancement effort for the rest of their lives.
Through peer support, encouragement and information, our communities help people
achieve excellent health outcomes.
Heart Support-Australia has been committed to establishing rehabilitation initiatives and creating support networks for heart patients for over 28 years.
Our Heart Health Support Service trains survivors who have ‘been there’ to offer encouragement, support and information to other
Australians with heart conditions.
We all have the power to help those in need.
When people join our community, they are surrounded by people who have “walked their path” – that is the basis of an effective peer support group.
Support from peers is valuable because it provides hope, role models and motivation to help patients manage their condition effectively.
